Evolution of Breast Cancer Management in the Last Four Decades

Since my days as a surgical intern in 1973 the approach to the management of breast cancer has evolved rapidly. This has been mainly due to the growing knowledge and better understanding of the disease that has taken place over the last three decades. Currently each patient´s individual breast cancer biology guides the treatment of the disease. Mastectomy is no longer routinely recommended.

Since medical treatment was introduced early in the management of breast cancer, we started seeing the progressive improvement in patient survival. We are now in a phase of fast growing drug development, offering target
therapies that are less toxic and better tolerated. Today a multidisciplinary team needs to see the patient at diagnosis in order to optimise disease management and quality of life. Medical treatment prescribed before breast surgery offers advantages and these will be presented.
